Required Online Feedback Form
If you are not a Missouri 4-H volunteer, please do NOT take the Feedback Form/Quiz.
Volunteers wishing to complete a print version of the feedback form should obtain a copy from the local 4-H faculty or staff.
Completion of the entire online feedback form provides verification that you took the volunteer orientation.
Instructions for online feedback form
- You are required to complete the confidential feedback form in one single, session.
- You should plan about 30 minutes to take the feedback form. It is not timed.
- Use the [Next] and [Previous] buttons to move through the feedback form.
- DO NOT use your internet browser's [Back] button or you may end up losing all your answers entered up to that point!
- After submitting your feedback form, your web browser will be directed to the Missouri 4-H Center for Youth Development web site.
